Youth Fall Baseball & Softball Sponsorship Agreement Form

The sponsor agrees to furnish shirts, caps, catcher's equipment*, bats*, and pay a sponsorship fee of $100 per team. For any new purchase of uniforms, Parks and Recreation requests that only shirts and caps be purchased.
Poplar Bluff Parks and Recreation will furnish batter’s helmets, bases, umpires, scorekeepers, supervisors, and other necessary personnel and equipment for games.
*Parks and Recreation offers catcher’s equipment and bats, upon availability (on a first come, first served basis). A sponsor/coach may reserve team equipment. A fee of $100 will be charged for equipment not returned.

Registration Deadline: July 21, 2023

Registration is not complete until payment is received.
